Linaclotide is a guanylate cyclase-C receptor agonist (GCCA) with both visceral analgesic effects and endocrine activity. Both linaclotide and its active metabolite bind to guanylate cyclase-C (GC-C) receptors on the luminal surface of small intestinal epithelial tubules. In animal models, linaclotide reduces visceral pain and increases gastrointestinal transit velocity through GC-C activation, and in humans, the drug also increases colonic transit velocity.The result of GC-C activation, i.e., an increase in intracellular and extracellular cGMP (cyclic guanosine monophosphate) concentrations. Extracellular cGMP reduces the activity of painful nerve fibers and can reduce visceral pain in model animals. Intracellular cGMP can increase chloride and bicarbonate secretion in the lumen of the small intestine by activating CFTR (cystic fibrosis transmembrane conductance regulator), which ultimately leads to increased secretion of small intestinal fluid and increased small intestinal transit velocity.
